Appointments can be made via telephone or in person. Please have your insurance information available for the secretary when you call. If for some reason the Center For Urology physician that you have requested to see is not available, we will try to accommodate your request at the earliest time available or schedule you with another physician. When you come to the office, please bring the following form(s):

• Female Patient Form

• Female Patient Form (Spanish)

• Male Patient Form

• Male Patient Form (Spanish)

  
  Our secretary will assist you in obtaining any x-rays or reports that may be required for your office visit. Your appointment will be confirmed one or two days prior to your visit with our office.
