At Center For Urology, patient care is our number one priority. We will work with you to schedule an appointment with the physician of your choice. If that physician isn't available we will suggest another qualified member of our group for you to consider.
When you call to schedule an appointment, please feel free to confidently disclose the nature of your medical problem, so we can ensure enough time for your visit.

Preparing for your Appointment
Appointments can be made via telephone or in person. Please have your insurance information available for the secretary when you call.
If for some reason the Center For Urology physician that you have requested to see is not available, we will try to accommodate your request at the earliest time available or schedule you with another physician.
When you come to the office, please bring the following information:
- A list of medications that you are currently taking
- Your insurance card(s)
- Any forms that were mailed to you by our office

New Patients
Please feel free to download and print a copy of our new patient paperwork. It would be appreciated if you could bring the completed forms to your first appointment along with your insurance card(s).
Many patients are more at ease filling out forms in their own homes. Should you have any questions, please feel free to call our office and we will be happy to assist you. The attached forms will help to ensure that your experience with Center For Urology has a positive start.
Our secretary will assist you in obtaining any x-rays or reports that may be required for your office visit.
Your appointment will be confirmed one or two days prior to your visit with our office.

Insurance/Billing Information
Center For Urology participates with most major insurance companies. Please contact your insurance provider to verify our participation.
While the fee is the patient's responsibility at the time of treatment, we will gladly assist you in filing insurance claims. Be sure to present your insurance information to our office staff at the time of your visit. Please be familiar with your insurance plan so that if referral paperwork is required, you will have it available before, or at the time of, your visit.
If for any reason your insurance carrier ultimately denies your claim and/or does not pay for the services rendered, you will be responsible for the bill you incurred for the services performed. For your convenience, we accept Visa, Mastercard, Discover and American Express.
